After filling in the required fields and hitting 'continue', a message box pops up pointing to my name that says 'Please match the requested format'. I've tried first & last names and my username, but neither work.

What browser are you using? You need to use Internet Explorer ver 8 or below OR Mozilla Firefox – Other browsers are not supported at this time.
